I am in the office between 9 am and 12 noon. I provide hospital services for my patients before and after that. I try very hard to see you the same day or next day for urgent visits.This may sometimes require a time slot before or after those hours and I will try to accommodate that. I schedule fewer patients so I can address all your current concerns and prior history adequately. It is very important to carry a current and accurate list of medications.
After hrs and Emergencies. Please call 911 for all emergencies A physician will always be available through the answering service for nonroutine issues. However, prescriptions will not be called in over the weekend.
Insurances and payment policies I participate with several insurance plans. Proof of Insurance is required at the time of visit. Copays are required by your insurance company at the time of the visit. However it remains your responsibility to check your plan will pay the physician. Please check/call the number at the back of your card to check your benefits. If you get a bill from us, it is because your insurance plan requires that you pay. Please call them first if you believe there is a problem. Please call our billing service if you believe there is a discrepancy. If you do not have insurance- payment is expected at the time of service. Failure to pay for 60 days after date of invoice may result in the submission of invoice to a collection agancy. The following are considered noncovered services by the insurance plans and are considered the patient's responsibility -telephone consults lasting more than 5 minutes ( if after 7 days of last visit) -returned check fee $10 -late fees $5/ month after the first 30 days -Copay collection fee- $5 ( if copay invoice has to be mailed to you) -Form filling fee if done other than at time of appointment $12. You may request these and pay via paypal -custom letters $20
Prescriptions. The
best time to get a refill is at the time of the office visit. If you
need refills outside of this visit please ask your pharmacist to
transmit OR fax a refill request. Certain prescriptions require periodic physical exams even if you feel completely well. Antibiotics will not be called in over the phone without a compelling reason. Please allow upto 2 days for the request to be filled. I do not prescribe medications for chronic pain. You will be referred to a pain management specialist if severe pain continues. Test Results I will call you oe email you with your test results when they are available. If you do not hear from us within a week of the test being done, please call the office. Do not assume that the results are normal. I also participate with Google Health and Quest Diagnostics and results can be made available within Google health PHR at your request..
